What if I don’t have a Medicare card?
For those patients without a Medicare card you will need to pay at the time of your visit, we accept either card or cash. You will be provided with a receipt and if you have private health insurance you will be able to claim a portion of the cost back.
Do I need to bring my Medicare card to my appointment?
Please bring your Medicare card and any concession cards you may have to each appointment.
Can I get my Medical Records transferred to your practice?
Ask Reception for a “Transfer of Medical Records Request Form”, fill it out and Reception will send it off to get your records for our files. If you are moving away and would like your records transferred, please fill out a form at your new GP’s and send it to us, and we will transfer your file promptly.
Can I have a phone consultation?
Our Doctors do not do consultations over the phone. If you have any health issues it is recommended that you make an appointment to see your practitioner. If you have a query for your practitioner, please leave a message with Reception.

Your medical record is a confidential document. It is the policy of this practice to maintain security of personal health information at all times and to ensure that this information is only available to authorised members of staff. We will communicate with you via text or email to advise you when clinically significant test results have been received or regular health checks are due.
You can opt out of this service at any time by advising us in writing.
